Basketball Handicap Betting: Spreads, Pushes and Overtime
Learn how basketball handicaps change the score for settlement, how plus and minus spreads work, when a push occurs, and whether overtime counts.
This guide explains spreads, pushes and overtime. It does not supply handicap predictions or accumulator tips. A line can make two teams look balanced for betting purposes, but it cannot guarantee either side will cover.
What does a handicap mean in basketball betting?
A handicap, also called a spread, creates an adjusted score used only to settle the bet. The favourite usually receives a negative number and the underdog a positive number.
If the market is:
- Home -4.5
- Away +4.5
you can calculate the two selections like this:
home handicap score = home final points - 4.5
away handicap score = away final points + 4.5
Compare the adjusted score with the opponent's actual score. The selected side must lead after the handicap is applied.
The minus sign does not mean the favourite starts the real game behind. The plus sign does not add points to the official league table. Both are settlement adjustments on the bet receipt.

For a negative spread, the required winning margin is the next whole point above a half-point line. A -7.5 selection needs an 8-point win. A -1.5 selection needs a 2-point win.
For a positive spread, the selection can lose by no more than the whole-point margin below the half-point line. A +7.5 selection can lose by 7. It loses by 8.
What happens with a whole-number handicap?
A whole-number spread can produce an adjusted tie. At Home -4, a home win by exactly 4 points creates a push in a standard two-way market.
bet365's current basketball rules say push rules apply to two-way markets unless stated otherwise. A push commonly returns the stake on a single bet. In a multiple, the pushed leg can be removed and the remaining selections recalculated under the operator's rules.
Not every product uses a two-way push. A three-way handicap can include a separate tie outcome. Some alternative lines or promotional products can also publish different settlement terms.
Half-point lines such as -4.5 and +4.5 cannot create a tied adjusted score when final team points are whole numbers. That removes the push but does not make the bet safer.
Does overtime count in a basketball handicap?
It often does for a full-game pre-match spread, but the receipt and operator rule must confirm it. bet365's published basketball rules say all pre-game bets include overtime unless otherwise stated.
FIBA's official rules use four 10-minute quarters and five-minute overtime periods when the score is tied. Other competitions can use different game formats. FIBA itself notes that NBA, WNBA and NCAA rules differ from the FIBA rules.
The betting question is separate from the competition format:
- full-game handicap including overtime: final score after played overtime;
- regulation-only handicap: score at the end of regulation;
- first-half handicap: score in the first half only;
- quarter handicap: score in the named quarter only.
Never transfer the full-game overtime rule to a half or quarter market.
Game, half and quarter handicaps use different score windows
A first-half -3.5 line applies only to the first-half score. A fourth-quarter +1.5 line applies only to points recorded inside the fourth quarter under the operator's period rules. The full game can finish in the opposite direction.
For example:
- Home leads 52-45 at halftime, so Home -3.5 wins the first-half handicap.
- Away then wins the full game 101-98, so Home -3.5 on the full game loses.
- Those are not contradictory results because the markets cover different periods.
Check what happens if the game is shortened, abandoned or not completed. A market whose outcome is already mathematically determined can be treated differently from a market that still depends on unplayed time.
How do player handicap matchups work?
A player matchup compares two named players and applies a spread to the relevant performance statistic. The bet365 rules state that spreads can be applied to each player's actual score and that push rules apply.
Suppose a points matchup is:
- Player A -2.5 points
- Player B +2.5 points
If Player A scores 24 and Player B scores 20, Player A's adjusted figure is 21.5 and the -2.5 selection wins. If they score 22 and 21, Player A's adjusted figure is 19.5 and Player B +2.5 wins.
Participation requirements matter. An operator can require a player to dress and see court time, and overtime treatment can depend on the exact prop rules. Do not use team-spread rules to settle a player matchup.
Is "any team to lead by 12" a handicap bet?
No. A team to lead by 12 points market asks whether the score margin reaches 12 at any qualifying moment. A handicap asks which side leads after a spread is applied to the score at the end of the stated period.
A team can lead by 12 in the second quarter and still lose the game. In that case a Yes selection on the lead-by-12 event can win while the same team loses a full-game handicap.
Read event markets literally:
- "to lead by 12" concerns a margin reached during play;
- "-12.0 handicap" concerns the adjusted period result;
- "winning margin 11-15" concerns the final margin band.
How to read the handicap price
At decimal odds of 1.91, the raw implied probability is:
1 / 1.91 = 52.36%
The Smarkets source explains this conversion. If both sides are priced at 1.91, their raw implied probabilities total more than 100%. The difference is part of the market margin. It is not proof that either team is incorrectly priced.
Changing from -4.5 to -3.5 improves the favourite's line by one point, but the price normally changes too. Compare line and price together. A better-looking spread at much shorter odds is not automatically better value.

Set a maximum stake before the game. A late score can change a handicap result without changing which team played better overall, and chasing that variance with a larger live bet increases loss risk.
